How Do I Sign Up?

Step One: Schedule Meeting

RPM services are done in collaboration with your doctor. Contact your doctor’s nurse to schedule a consultation meeting, alongside, your clinical pharmacist.

Step Two: Finalize Details

After agreeing to obtaining medication therapy services and remote patient monitoring with your doctor, your enrollment will be documented in your electronic health record (EHR). We’ll walk through and document’s important details together.

Step Three: Onboard RPM

With our help, you’ll provide any important information regarding your chronic conditions so we can begin to develop your care plan. If needed, this is when we’ll get you set up with your blood pressure machine and/or glucometer.

Step Four: You’re All Set

Your RPM results will be assessed every 30-days by your clinical pharmacist, and monthly follow-up meetings will be conducted. We’ll stay connected through the month and adjustments will be made to your medication therapy accordingly. 30-day results will be sent to your physician(s).

Enrollment into RPM grants you access to your own personalized platform accessible by web or mobile application. Equipment is used to monitor blood sugar and blood pressure that is sent directly to your clinical pharmacist.

Point-of-Care Lab Testing

Point-of-Care Lab Testing

$15 - $30

This service allows for periodic testing to be done to manage your medication therapy. Examples of tests done are HgbA1c, lipid panels, and INR monitoring for people on blood thinners.
